Grades 6-12 Carlton Parents, Your voice matters! Conversations and perceptions about youth substance use are important. Please take some time to fill out this survey (Grades 6-12 parents only). We will use the results of this survey to plan programs in our schools and our community in order to keep students safe and healthy. This survey is for parents and guardians of children in grades 6 through 12 in Carlton Public Schools. If you have more than one child in the grades being surveyed, please answer all questions by thinking about the child with the most recent birthday.
